• Must have prior telemetry experience

Job Requirements

• Proof of 2 immunizations, positive titer or Declination accepted for MMR.
• Must have Fit Test completed within 1 year prior to start, updated annually.
• Proof of 3 immunizations, positive titer or Declination accepted for Hepatitis B.
• Influenza Vaccine required during flu season with accepted medical/religious exemptions.
• Proof of negative TB test (QuantiFERON Blood Test within one year or 2-Step TB Skin Test) or documentation if positive.
• Health Statement signed and dated within 1 year stating ability to work without restrictions.
• Proof of 2 immunizations, positive titer or Declination accepted for Varicella.
• Proof of Tdap vaccination within past 10 years or Declination accepted.
• Annual TB Questionnaire required within 6 months of start with documentation if positive.
• Negative 10 Panel Drug Screen within 30 days prior to start.
• Unit specific skills checklist within one year prior to start, updated annually.
• Competency exam within one year with passing score of 80% or higher.
• Core competencies current within a year prior to start with passing score of 80% or higher, updated annually.
• Eye Exam & Color Vision test within 1 year prior to start.

Schedule Information

• Contract length is 13 weeks.

• Unit specific skills checklist required within one year prior to start, updated annually.

Additional Details

• COVID-19 Vaccine proof or exemption accepted.
• FMOLHS Resume required for submission.
• FMOLHS Criminal Background Check must be completed within 30 days prior to start with detailed requirements.
• FMOLHS HRIS Form required for submission.
• FMOLHS Badge Photo required.
• FMOLHS Contract Annual Education Packet required.
