Job Summary:
Under general supervision, the pharmacist will provide appropriate drug therapy to adult and pediatric patients by screening physician's orders thoroughly, accurately entering the orders into the computer system, monitoring patient drug therapy, recommending appropriate changes in medication, and compounding and dispensing drugs without error. The pharmacist will also provide direct supervision of all technician duties and act as a drug information resource person for all health care professionals and patients.
Clinical Practice:
- Provides appropriate drug therapy by screening physician's orders entry thoroughly, accurately enters orders in the computer system, monitors patient's drug therapy, recommends appropriate changes in medication.
- Reviews patient's charts and related information systems for diagnosis, drug history, allergies, BSA (when necessary), procedures, and complications.
- Follows-up on all omissions, discrepancies, and therapeutic duplications.
- Completes an accurate computer profile for each patient.
- Prioritizes clinical responsibilities in order of importance.
- Selects cases and monitors patient's lab and test results; makes recommendations for changes in drug therapy; monitors cumulative dose for selective drugs.
- Documents several times a day clinical interventions in the patient profile.
- Applies knowledge of basic drug regimens and related disease states for which they are used.
- Reviews all new protocols in the assigned area when needed.
- Applies knowledge of all components of active investigational protocols as needed.
- Participates in the development of research projects (e.g., pharmaceutical background, drug information, new procedures) when requested.
- Maintains and identifies unused/missing doses and resolve discrepancies on a daily basis.
- Initiates appropriate practice regimens according to MSKCC guidelines (e.g., antiemetic, chemotherapy, epoetin alpha).
- Understands and applies as needed the Investigational Drug Process (e.g., can describe the four-phase process of drug development, function of IRB, etc.).
Order Processing:
- Accurately screens and enters orders in the patient profile.
- Compounds and dispenses drugs correctly.
- Makes accurate entries of every order to complete profiles and labels including patient name, drug, dose, fluid volume (if administered via IV infusion), special instructions and/or precautions; updates allergy, height, and weight sections.
Category I:
- Acceptance as an author/co-author for a presentation at a national, state, or local Pharmacy or Allied Health professional meeting.
- Acceptance for publication as an author in a pharmacy or national allied health professional journal (e.g., AJHP, J Clin Oncology, etc.).
- Elected office held in a professional health care association or organization (e.g., ASHP, NYSCHP, ASPEN, etc.).
- Co-Investigator in an IRB-approved protocol.
- Completion of an ASHP Board Certified Specialty.
- Completion of two graduate school courses, which meet with MSKCC tuition reimbursement criteria. Copy of school transcript required.
- Author for an article in Pharmacy Newsletter (FOCUS) publication.
- Presentation at a Pharmacy Core Course.
Category II:
- Inservice presentation to pharmacy, nursing, or medical staff with written documentation (Attendance sheet and outline).
- Completion of one (non-graduate) school course, which meets MSKCC tuition reimbursement criteria.
- Involvement with a voluntary ongoing departmental committee or special project (requires 2 meetings).

Required Education:
-Bachelor's Degree in Pharmacy from ACPE-accredited school.

Required Certifications & Licensure:
- A valid NYS Pharmacist License.
- Pharmacists who are licensed in another state but are awaiting licensure for the current state they are employed must work under the supervision of a licensed pharmacist.
